Payment Policy & Patient Responsibility

It is our policy that patients are responsible for all charges resulting from medical and surgical treatment provided by Cornell Dermatology. Any and all payments are due at the time of service unless other customized arrangements have been made with the billing department. All invoices from Cornell Dermatology are due upon receipt.

Out-of-Network Care and Insurance Billing

Cornell Dermatology's primary physician, Dr. Sandhya V. Koppula, is a non-participating provider as of January 1, 2020. It is the patient's responsibility to understand his or her out-of-network benefits. Cornell Dermatology will not bill patients' primary or secondary insurance. However, we will provide a customized receipt on the same day of a patient's visit to assist in the patient's self-billing process. It is the responsibility of the patient to provide his or her correct insurance information in the event that any laboratory work is needed.

Appointment Policy

We expect patients to arrive at least five minutes early to their scheduled appointment to accommodate check-ins. We expect new patients to arrive at least twenty minutes early to their scheduled appointment to accommodate new patient paperwork. If a patient is more than 10 minutes late, the patient will be considered a no-show and will need to be rescheduled. Any patient who fails to arrive at his or her appointment without cancelling at least 24 hours in advance of the scheduled appointment is considered a no-show.

Without prejudice to our other rights under these terms and conditions, if you breach these terms and conditions in any way, we may take such action as we deem appropriate to deal with the breach, including suspending your access to the website, prohibiting you from accessing the website, blocking computers using your IP address from accessing the website, discharging your patient record, contacting your internet service provider to request that they block your access to the website and/or bringing court proceedings against you.

Cornell Dermatology is permitted to revise these Terms and Conditions at any time, and by using this website you are expected to review these Terms and Conditions on a regular basis.

Cornell Dermatology may assign, transfer, sub-contract or otherwise deal with its rights and/or obligations under these Terms and Conditions without notifying you or obtaining your consent. However, you may not transfer, sub-contract or otherwise deal with your rights and/or obligations under these Terms and Conditions.

If a provision of these Terms and Conditions is determined by any court or other competent authority to be unlawful and/or unenforceable, the other provisions will continue in effect. These Terms and Conditions will be governed by and interpreted in accordance with the laws of the State of Oregon, and any disputes relating to these Terms and Conditions will be subject to the exclusive jurisdiction of the courts of the State of Oregon.
